The Official Tournament Entry Form and the USA Softball Official Championship Roster must be completed and returned to the tournament director, along with the entry fee of $600.00 (made payable to USA 16U Nationals). The forms MUST be signed by your State Commissioner or his/her designee. Your local USA Softball Commissioner or his/her designee will be the one to supply this form. The deadline for this form and entry fee to be received is July 12th at 5:00pm. These documents can be mailed or submitted via email. The Pick-Up Player Form is only applicable to teams that have pick up players. The form MUST be signed by your State Commissioner or his/her designee and can be brought to the team check in.
